About Transitions @ Plymouth Center

The Transitions @ Plymouth Center Program is a community-based program specifically tailored for special education students 18-22, who will receive a certificate of completion after attending a four-year transition program post-High School. These students typically receive accommodations and support to access the curriculum. Situated in downtown Plymouth, our students have the opportunity to build independence in accessing the community, utilizing public transportation, and engaging in work & volunteer experiences. Program goals focus on fostering each student’s development of self-advocacy, interpersonal and independent living skills,  engaging in person centered planning and functional academics allow our young adults to become more active members in their homes and communities.  At least 1 day/week students are on the BSU campus, where many of our students get a taste of “campus” life alongside same-aged peers.   Students who are enrolled in the  Transitions @ Plymouth Center Program possess the capability and desire to achieve tasks with as much independence as possible, such as navigating their community, running errands, attending classes, and participating in supported work environments 

Individualized Transition Instruction & Community Engagement 

• Functional academics & hands on opportunities designed to enhance real-life skills – meal planning, shopping, cooking, budgeting 

• Small PACkages – a student developed and run small business 

• Volunteer, internship and work experience  

• Certification & Training Opportunities (CPR First Aid, ServSafe, SALs, pre-ETS) 

• Travel training- students learn to navigate a variety of community locations and the BSU campus safely 

• Self Determination- students participate in person centered planning, goal setting and are an active part of their IEP Process and post-secondary planning 

• PAC staff work closely with LEAs and families as they navigate the transition process partnering with state agencies such as MRC and DDS  

College Partnership 
PAC is a Partnering District with Bridgewater State University affording student the opportunity to participate in the Self-Advocacy Leadership Series (SALS), the Transitions at Bridgewater (T@B) Program, and if accepted through the application process, BSU’s highly sought after EXCEL program.
