PAC Transitions @ Silver Lake Middle School is specifically designed to support students ages 18-22 with significant and complex needs as they begin the process of transitioning from school to adulthood. Our program focuses on developing essential skills in communication, adaptive behavior, daily living, and functional academics. We aim to empower students to reach their fullest potential by providing them with meaningful experiences and opportunities that enhance their independence and to prepare them for entry into adult services.

Program Features:

  • Individualized Support: Each student receives personalized support tailored to their unique strengths and needs.
  • Comprehensive Team: Our dedicated staff includes a Transition Specialist/Special Educator, Board Certified Behavior Analyst (BCBA), Speech-Language Pathologist (SLP), Occupational Therapist (OT), Physical Therapist (PT) and onsite nursing. This multidisciplinary team works together with our Educational Support Professionals (ESPs) to ensure each student receives individualized, comprehensive and coordinated support.
  • Supported Volunteer Opportunities: We offer students carefully selected and supported volunteer opportunities that match their interests and capabilities. These experiences are structured to provide a sense of contribution and belonging while developing foundational skills.
  • Skill Development Areas:
    o Adaptive Behavior: Structured support to help students adapt to new environments and routines, with an emphasis on consistency and predictability.
    o Daily Living Skills: Emphasis on developing basic self-care routines and skills, using visual supports, step-by-step instructions, and hands-on practice.
    o Functional Academics: Customized learning experiences that integrate sensory and experiential activities to enhance understanding and engagement.
  • Transition to Adulthood: Our program addresses all essential components of transition planning, with a focus on building self-advocacy skills, exploring post-secondary options tailored to individual needs, and developing independent living skills at a level appropriate for each student. We are dedicated to fostering a seamless transition to adult life by collaborating closely with families, caregivers, and adult community service agencies.

Community Partnerships/Experiences
– Active engagement within the community via regular community trips and volunteer sites
– PAC is a Partnering District with Bridgewater State University affording students the opportunity to participate in the Self-Advocacy Leadership Series (SALS) and the Transitions at Plymouth Center (T@PC) Program

Student Focused Transition Support
– Self-Advocacy – students learn to make independent choices about their day and activities
– Self Determination- students participate in person centered planning, goal setting and are provided the support to be an active part of their IEP Process
